At your First Visit
The dentist will:
Listen to your dental concerns,
Undertake a comprehensive examination
Undertake required X-rays, scans and photographs
Provide a professional diagnosis
Provide an itemised treatment plan and costings for your dental treatment.
To ensure the best possible dental care, an understanding of your medical conditions and any regular medications will be required.
You will be asked to complete a ‘Medical History’ form upon your arrival.
The dentist will go through this information with you during your appointment.
Please bring a list of the medications that you are taking.
If you have been regularly seeing another Dental clinic, you may be asked to sign a ‘Release of records form’ so that a copy of your previous dental treatment and associated x-rays can be obtained.

HICAPS available for on-the-spot processing of Private Health Fund rebates on the day of treatment.
SV DENTAL are NOT a preferred provider (except HCF), however, they accept all Private Health Funds.
Government Dental Clinic Vouchers accepted.
DVA (Dept Veteran Affairs) patients accepted.
CDBS (Child Dental Benefit Scheme) eligible patients accepted.
